Who do I see?

Routine appointments

The easiest and most efficient way to book an appointment is via the NHS App.

Alternatively, you can contact us online or call the surgery on 01772 970150.

If you are unable to attend an appointment at the surgery, we would be grateful if you could please let us know, as soon as possible.  This will then allow the appointment to be offered to another patient and reduce waiting times.

Pharmacy First Scheme

People in Lancashire and South Cumbria are able to access medication for a number of minor ailments without the need for a prescription.

Under the minor ailment supply service, people suffering from a number of specified conditions can receive the treatment they need from their pharmacy following a referral from their GP practice.

In order to access the service, people still need to call their GP practice, but the practice will be able to refer them straight to the pharmacy of their choice, rather than require them to attend an appointment in their practice.

Conditions for which people can access the appropriate medication under the minor ailment supply service include:

  • Bites/stings
  • Colds
  • Congestion
  • Ear or eye complaints
  • Hayfever
  • Thrush
  • Skin conditions, such as acne or athlete’s foot
  • Mouth and throat issues, such as cold sore blisters
  • Swellings, such as of the ankle, foot or lower limb.

If you do not pay for your prescriptions, you will not pay for the medicines supplied.

NHS Lancashire and South Cumbria Integrated Care Board, which plans and manages health and care services across the region, has launched the service to allow those suffering from minor ailments to receive faster, more appropriate support and free up much-needed GP appointments for patients with more complex needs.

Accessible Information Standards

There are five key requirements of the Standard and as a Practice we will:

  1. Ask patients and carers if they have any information or communication needs, and find out how to meet their needs;
  2. Record those needs in a set way;
  3. Highlight the patient’s file, so it is clear that they have information or communication needs, and clearly explain how those needs should be met;
  4. Share information about a person’s needs with other NHS and adult social care providers, when they have consent or permission to do so;
  5. Make sure that people get information in an accessible way and communication support if they need it wherever we possibly can do.

Here at the Park Medical Practice we will provide all our patients access to information they can understand and offer any support needed if a communication need is highlighted.

Enhanced Access Appointments

Enhanced Access is a serviced commissioned by Lancashire & South Cumbria Integrated Commissioning Board (ICB) to provide a seven day service to patients. We are working in collaboration with the following practices to provide an out of hours service:

Lane Ends Surgery ( Doclands Medical Centre)

Kingsfold Medical Centre

New Hall Lane Surgery

The service is available Monday to Friday 6.30pm – 8.30pm, Saturday 9.00am – 5.00pm and Sundays 9.00am – 12.00pm. There will also be a Bank Holiday service (with some exceptions) but the times of this may vary.

Appointments are a mixture of face to face and telephone consultations, and you may be asked to travel to a different practice to be seen. As well as GP appointments, staff at the locality clinics will provide a range of appointments with Practice Nurses, who will be available to provide chronic disease reviews, smears and childhood immunisations, along with other Health Care Professionals.

Our staff can book these appointments for you, as we would for our weekday service. When the service is open our phone lines will divert automatically to the service, but you can also ring the Increased Access Service directly on 01772 970150.

Unfortunately, there is no facility to book or cancel appointments online for the Increased Access Service at this time.

Please note that once all availability is full, you may be directed to an alternative service, such as 111.

Please also be aware that the administrative staff working at the locality surgeries do not have access to patients’ full medical records and therefore will not be able to answer queries on referrals, prescriptions, test results, private work and entries to medical records.

Remember: In a genuine emergency, you should call 999.

Chest pains and / or shortness of breath constitute an emergency

Home Visits

Park Medical Practice offer a Home Visiting Service to all those patients who are housebound and are unable to attend the Practice.  We would appreciate if you could ring the Practice as early as possible so that we are able to arrange a visit on the day.  Please provide the Care Coordinator with detailed information regarding the home visit as this will enable the clinician to prioritise the visit schedule.

Please remember that there are better facilities for examining and treating patients at the practice. Home visits should only be requested for patients who are permanently housebound because of illness or disability, or are genuinely too unwell to come to the surgery on the day.

A member of the clinical team may telephone you in advance of visiting to gain a better understanding of your problem and situation and may decide that a visit to your home might not be necessary as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send another healthcare professional, or indeed arrange a hospital attendance.
