ASTHMA & COPD SERVICES
Respiratory clinics are run appropriately by trained Practice Nurses to provide help with the care of Asthma and Chronic Obstructive Pulmonary Disease (COPD) in terms of education, preventative measures, drugs and general management of the illness.

To ensure your appointment is productive and your review completed on time, we would recomend if you could please complete Asthma control test - adult.pdf or if you have COPD then please complete COPD Assessment Tool and email it back to us prior to your appointment on:
DIABETIC CARE SERVICES
Most patients with Diabetes are now managed within the Practice (including those on insulin). The Practice aims to see all its Diabetic patients on an annual basis to assess their ongoing treatment and support. Patients will be asked to attend a screening appointment initially for blood tests and a foot check followed by an appointment with a nurse to discuss the blood test result and agree a management plan until the next review.
NHS HEALTH CHECK
The NHS health check is a health check up for adults aged 40-74. It is designed to spot early signs of stroke, diabetes, kidney disease, heart disease and dementia.

International Normalised Ratio (INR)
The practice offers a ” one-stop” shop for stable patients using anti-coagulant therapy, if you fit this criteria then you are able to attend the clinics run by our Health Care Assistants and Clinical Pharmacists.
Patients should contact the surgery for more information, our clinics run on a Tuesday and Friday morning by appointment.
