HOW TO ORDER YOUR PRESCRIPTIONS

ONLINE:

The safest and most secure way to order your prescription would be doing this online via the NHS App https://www.nhs.uk/nhs-app/. You would need to download the NHS app and provide your NHS number and some photographic ID.

Prescription Order Slip:

Please visit the surgery and a member of the Reception Team will be happy to print your prescription list for you to complete and drop into the Prescription box, located in the entrance porches at both sites. Alternatively these can be posted via Royal Mail to Park Medical Practice, Cottam Lane, Ashton, Preston, PR2 1JR.

These medicines can potentially cause serious side effects, need regular monitoring or are Controlled Drugs [CD] whose use is governed by law because of the risk of drug dependence or abuse. For these reasons, the prescribing and supply of these medicines needs to be monitored to ensure patients do not suffer any harm as a result of their use.

Medication requiring monitoring will;

NOT be placed on your repeat prescription. The medicines will not be visible on paper copies of your repeat prescription and will not appear online in the list of repeat medicines available to order. It will also not be possible to obtain these medicines from your Pharmacy using repeat or "batch" dispensing.
You will need to complete monthly requests for these medicines. This can be done using paper request forms available at Reception, through prescription line or via the MYGP app by adding a message.
A GP or a member of the pharmacy team may need to contact you before issuing your prescription. It is therefore essential that you provide the surgery with up to date contact details including your mobile phone number.
If regular bloods tests are needed (indicated by a * against the medication listed above ) to monitor the effects of your medicines, a Doctor will not be able to issue your medication until the results of these tests are available to the practice. Please have any blood tests done promptly before you are due to run out of your medication.

Please remember to order your medicines in good time. As additional checks may need to be carried out, it may take longer to provide prescriptions.

Collecting your prescription

All medication is reviewed regularly by a clinician or clinical Pharmacist. At times you may be asked to attend the surgery for an appointment to review your medication. Please allow a minimum of two full working days before collecting your prescription from your norminated pharmacy.
